Visitor COVID-19 Testing

All visitors, vaccinated and unvaccinated upon entering the facility will be tested for COVID-19 as follows:

• Vaccinated visitors will be test once per week
• Unvaccinated visitors will be tested twice per week

The Rapid Antigen Point of Care (POC) test will be administered by our HCP or by yourself. This test is rapid and takes approximately 15 minutes so please plan on waiting for results.

Proof of Vaccination

The following guidelines will apply when asked to provide proof of vaccination.

Pursuant to the CDPH Guidance for Vaccine Records Guidelines & Standards, only the following modes may be used as proof of vaccination:

1. COVID-19 Vaccination Record; OR
2. A photo of a Vaccination Record Card as a separate document; OR
3. A photo of the visitors Vaccination Record Card stored on a phone or electronic device; OR
4. Documentation of COVID-19 vaccination from a health care provider; OR
5. Digital record that includes a QR code that when scanned by a SMART Health Card reader displays to the reader client name, date of birth, vaccine dates and vaccine type. The QR code must also confirm the vaccine record as an official record of the state of California; OR
6. Documentation of vaccination from other contracted employers who follow these vaccination records guidelines and standards.

Health Care Personnel/Staff & Volunteers Testing

Health Care Personnel/Staff & Volunteers will be tested for COVID-19 weekly by administering the following the Polymerase Chain Reaction (PCR every week)

We will continue to make adjustments as necessary. This schedule may be subject to change in order to ensure the safety of our residents, health care personnel and visitors. We continue to encourage outdoor visits, which are resuming in the Ebensteiner Garden or if you prefer visiting in the front of the building, near the flag pole, please let us know.

Social Media and Compassionate Care Visits

We continue to have FaceTime calls, social media visit calls and end-of-life and compassionate care visits, which are on a limited time per visitor and per visit.
